Moruya Medical Centre is seeking a VR GP in our growing Associate Medical Practice on the NSW South Coast. The busy practice currently has 7 GP's, Registrars, medical students and full time nursing cover. Position commitment includes a shared VMO Roster for on-call cover.

Title: Chronic Disease Practice Nurse

Provide supportive clinical care within the context of a primary healthcare setting and be part of a larger multi-disciplinary team with a focus on improving health outcomes for our patients.
The chronic disease nurse has a defined role that addresses key responsibilities in the area of patient focused clinical care, chronic disease management, preventative health and education.
The chronic disease nurse will provide a supportive and complementary role to the General Practitioners of this Practice and to our presenting patients, providing a variety of services that enhances the quality of patient care.
The Practice Nurse is required to:
Manage patients that have been identified as having a chronic disease, including working closely with the General Practitioner around the preparation of management plans, referrals to allied health services, referrals to health programs and regularly review and maintain the recall register.
Manage coordination of patients with the multi-disciplinary team, including Allied Health Services and other external service providers.
Organise and refer patients for Home Medicine Reviews and support patients who attend the Practice for medication management.
Case management of complex clients, crisis support and advocacy.
Preventative health activities including health assessment, immunization and opportunistic health education.
Demonstrate a comprehensive and accurate knowledge and skill in providing care that is responsive to individual and group circumstances:
Conduct accurate comprehensive assessment of individuals and groups presenting with or without appointments to improve outcomes and minimise adverse events.
Identify and provide comprehensive physical, psychosocial and emotional care for individuals and groups.
Provide health care services in accordance with Medicare Benefits Schedule conditions.
Assist with minor procedures undertaken by the General Practitioner.
Initiate and conduct comprehensive health promotion and education by promoting patient, carer and community well being through health information, education, specific programs, community development and self-care. As well as providing timely, accurate information and education to staff and medical students within the Practice.
Undertake health screening and health monitoring activities including ECG and spirometry, as well as other clinical tasks requested by the General Practitioner.
To sustain this practice by contributing to better management of human and material resources through optimising the use of professional resources, building practice capacity to adapt to change, and to maximise financial efficiency.
Always use the best available research to inform clinical care management. Collaborate with the Southern NSW PHN, General Practice and health product provider representatives to access current information. Initiate changes to practice guidelines and protocols based on evaluation of research outcomes and evidence. Participate in research taking place in the general practice setting.
Monitor infection control processes in the Practice including compliance with sterilisation guidelines. Maintain the cold chain systems and other medicine storage systems. Participate in the accreditation process. Assist with triage and acute presentation support. Assist with minor procedures and wound care.
Ensure clinical nursing decisions are communicated to the general practice team. Appropriately refer clients to general practitioners. Collaborate with general practitioners to develop guidelines and protocols. Attend and contribute to practice meetings.
Recognise the need for ongoing education and training. Maintain skills in CPR, immunisation and other first aid requirements. Adhere to all policy and procedures as set out in the Moruya Medical Practice Policy and Procedure Manual.
